MEMBERSHIP

This membership begins on the date of this Agreement and will continue, for a 12-month period or a monthly basis depending on the selection indicated on this form. The membership will be automatically renewed on an annual or monthly basis depending on the members selection (the “Renewal Date”);

The Club will provide annual members with written notice by mail or email, to the mailing address specified by me in this Agreement, of the renewal of the Agreement approximately 45 days prior to the Renewal Date.

If transitioning to monthly billing, please note that any requests for this change must be processed before your annual renewal date.

Once the annual dues are charged to your account, you will not be able to switch to monthly billing until your renewal period concludes in a year’s time.

Annual members will automatically renew upon the annual renewal date. Rates are subject to change upon renewal with 30 days notice from management. I may renew the annual membership by the terms set out herein (and at the current membership rate) by making payment for the following year; If I do not wish to renew the Agreement, I must give notice of cancellation 30 days before my annual renewal.

Annual Membership will receive 4 guest passes for guests to use the athletic facilities for the day. Please note that additional guest passes are $25 per pass, payable by either the member or the guest. If you do have someone who you think has an interest in joining the Club, just reach out and we would be happy to provide you with an additional pass.

Monthly memberships will automatically renew on a monthly basis and are subject to change with 30 days notice from management. If I do not wish to renew the Agreement, I must give notice of cancellation 30 days before my successive monthly renewal. The Club is not required to provide me with notice for renewal of a monthly membership.

Please note: those who are part of a Corporate membership are obligated until their Renewal Date. If I elect to renew on the Renewal Date, all membership fees are owed by me for the next 12-month period. If I leave the company or am transferred, another person from my company may join the Club and use any remaining portion of my membership. I may continue as an individual member at the regular posted rate.

The services and the prices for the services provided by the Club are set out in the Membership Package provided to me, the Applicant, at the time of signing of this Agreement and which is considered Appendix “A” to this Agreement. The Club will give notification of fee increases in writing, at least 30 days prior to said increase taking effect.

If I do not wish to renew the Agreement, I must notify the Club in writing 30 days prior to the Renewal Date. Any initiation fees are non-refundable. If I decide to cancel my membership before the first annual renewal date, there will be a $500 cancellation fee.

Locker contents emptied due to cancellation of my membership with be stored at my risk for a 30-day period, after which they will be donated to charity. Early cancellation of membership will be reviewed by management on a case-by-case basis with legitimate cause i.e. termination of employment, relocation, or health related issues. 30 days written notice as well as proof of cause is required.

Membership dues are non-refundable, except in extenuating circumstances, subject to managements judgment.

An alternative to cancellation, is to put membership on hold, and to extend the yearly renewal date. Putting membership on hold is subject to a $800 fee, and can be put on hold for up to 12 months.

To abide and be bound by rules of the Club publicly posted hereafter from time to time at the premises of the Club and that I acknowledge that injury or morbidity may result from physical exercise and from playing sports. To conduct myself in a manner that is respectful to all property, personnel and other members of the Club.

Save and except in the case of gross negligence, the Club, its officers, directors and employees shall not be liable to me, or to anyone claiming through me, for any claims, demands, injuries, damages, actions, or causes of action whatsoever and howsoever arising, and whether with respect to personal injury or loss of personal property or otherwise;
